1. How does the Option One Discounted Dental
Program work?
Option One is a dental program that
offers significant discounts on dental care through its network of providing
dental offices. The following is a
list of highlights about the Option One Dental Program:

Within approximately 2 weeks of
receiving your payment, a letter along with your membership card will be sent to
your home.
2. Does this program differ from
insurance?
Option One program is not insurance!
There are no benefits payable to participants, nor does Option One
compensate providers for services they may render to participants. You are responsible for paying the
discounted fee directly to the participating provider, at the time you receive
services.
3. Can participation in this program be
denied?
No!
There are no restrictions on participation in Option One. Treatment for pre-existing dental
conditions will be discounted.
4. Who can I enroll?
If you
select family enrollment, all members of your immediate family, defined as your
spouse and your daughter(s) and son(s)(up to 19 years of age), currently living
in your household can be enrolled. Any new family members can also be
enrolled. If you enroll for single
coverage and want to enroll family members later on, you must change your
enrollment selection.
5. Can I receive discounts before I receive my ID card?
Yes,
as soon as we receive your payment you are active. Your membership card
will be mailed within approximately 2 weeks of receipt of your
payment.
6. How often can I use the Option One
program?
There
are no limits on how often you and your family can use the Option One program,
as long as the ID card is valid.
7. Am I required to pay providers when I receive services?
Yes,
payment is due at the time of service, unless you have made different payment
arrangements directly with the provider.
8. What if a participating provider does not recognize my
Option One card?
If a
participating dentist does not recognize your Option One card, simply ask the
provider to contact a customer service representative for Option One at
1-800-500-7760. The customer
service representative will supply the necessary information to your dentist.
9. Do I have to change my dentist? Can I go to a dentist who
is not in the network?
Option
One discounts are available only when visiting a participating dentist. If your
current dentist participates in the provider network, you simply need to present
your ID card to receive instant discounts. If not, you may wish to select a
dentist who does participate in the program so that you can take advantage of
the discounts.

near your home address participate in Option One! Either choose from

are three ways to enroll:
1. Internet
2. Over
the telephone by credit card. Call one of our customer service representatives

3. Pamphlet/Brochure. If you
have received a brochure via an employer group, mail, referral, marketing
campaign, association, etc… please fill out the enrollment form that is included
as part of the brochure, and mail it, with payment, to the address listed on the
back of the brochure.
